Studies on management of suicidal patients


Abstract
Hospitalization of the suicidal patient must be regarded as only one aspect of the total treatment program of that patient. When it is indicated, hospitalization is essential and should be instituted promptly. On the other hand, the period of hospitalization should not be unduely prolonged and should include active planning for the posthcspital phase of treatment. In discussing treatment of the suicidal patient, Litman emphasizes as fundamental that a psychotherapeutic relationship be established and maintained consistently during the suicidal crisis, so that the patient is able to appeal for help and so that the therapist is able to perceive and respond to the appeal until the suicidal urges have abated.
The authors studied the man-agement of 464 suicidal patients who were referred to Hangang Sacred Heart Hospital, Chun-g-Arg Unieversity, College of Medicine from January 1973 to December 1974.
The results were as follows:
1. Of the 464 cases of attempted suicides that entered emergency room, 405 cases (87.3%) were consulted to internal medicine, 32 cases (6. 9%) to neuropsychiatry and 8 cases to another departments.
2. Among them, 263 cases (56.7%) were admitted and the number of cases admitted to internal medicine was 246, to neuropsychiatry 9 and to another departments 8.
3. Of the 246 cases admitted to internal medicine, 150 cases (60. 9%) were discharged within three days and 35 cases were consulted to neuropsychiatry among whom 12 cases were transfered.
4. Average duration of hospitalization of the suicidal patients was 3. 9 days in internal medicine, 8. 3 days in neuropsychiatry and 7. 4 days in another departments and then that of those cases who were transfered from internal medicine to neuropsychiatry was 25.9 days.
5. Of the total cases of attempted suicides, the patients who got neuropsychiatric consultation were 9.5%.
